Players Advantage Club members are eligible to receive a Win/Loss Statement by mail or pick up. Please click on the link at the bottom of this section and print out the attached document. Complete the form and mail or fax to 510-620-2603 San Pablo Lytton Casino The information you provide on the form must match that of your account in order for a statement to be mailed to you. The Win/Loss Statement will reflect our records of your card-in play for the year you request as of the date indicated on the form.
